Just back from a very cold long ride, with a pretty heavy cold too and I would have killed for a warm drink.
What are people’s opinions on any of the thermal bottles? Do they actually work, for any length of time?
Any recommendations? Must fit in a bottle cage, not an anything cage, rucksack etc

These are quite good. I expect they'd fit in a bottle cage maybe with some padding if a bit loose:

I have one of those thermal mugs. It won't die because it's a very simple little thing, but very robust. I have wrapped mine with duct tape, fits in a bottle cage nicely now.

My Zojirushi is too posh to put in a bottle cage so that's rucksack only!

been living in Aus and opposite problem but when lived in Yorkshire and for long sub zero winter rides hot blackcurrant in one of those lifeventure screwtop mug thingies ^^^^ worked for me

I’ve used a camelback podium insulated bottle, it won’t keep your tea hot like a thermos flask, but fill it up with piping hot orange juice and it’s enough to give you some warmth for an hour or so.
