how does the rear cable get routed on the rear mech?
on the clamp plate thingie there is a lug that fits into a notch in the rear mech but the cable would have to do a 90 degree bend to get under this
or do I keep the cable straight and not put it through this lug?

The lug is there to stop the plate turning when you tighten the bolt, just run the cable under the tab that sticks out(sometimes there's a groove cut into the mech for it too).
