My gear changes on certain gears seem lazy and require 2 presses of the shifter sometimes not all gears though. I suspect cable stretch(??) maybe. I understand the barrel adjuster on the shifters takes up the slack....so looking at front of shifter what direction do i turn it and by how much?

I do have a worn chain(according to the gauge) is that the prime issue or just stretch in cables?.

have a look here, if it still doesn't work then suspect wear.

[edit] if the chain is worn enough for gauge to suggest replacement then I'd be lookign at chainrings and cassette very closely and expecting to replace those too in the near future.

Looking towards the shifter you turn the adjuster anti-clockwise - ie undo the screw to take up the slack. This will move the little metal part inside outwards (unscrewing it) and effectively put the cable under more tension.

There's a good chance you might need new cables (inner and outer) How long is it since they were replaced? Are the outers continuous or split?
Easy and cheap job for someone that knows what they ae doing (Me ๐Ÿ˜‰ ) but could cost a small fortune in a bike shop.....

ummm...replaced in may/june/july iirc. only exposed cable is last 20cm of front mech cable. cassette looks fine no obvious wear. chainrings are ok outer is quite worn i guess but its only rear shifts that are lazy mostly in mid cassette.
