    Ultrasound is usually first line imaging and that can often ascertain how doablean injection might be and if there is inflammation of the associated bursa.

    Be prepared to have your feet squeezed and lots of gel between the toes!

    Having suffered with IBS type symptoms and diagnosed such by a gastroenterologist I had spent the last 20 years or so getting on with it. The diarrhoea, bloating etc. I just thought that was my lot in life. I read a little bit about FODMAP a few years ago and saw some culprit foods that I really enjoyed.

    I at loads of broccoli and cashews as part of what I thought was a healthy varies diet.

    Since stopping eating those my guts have been transformed, instantly the diarrhoea, bloating and cramps stopped.

    20 years” frontline” NHS service tells me that whilst there are highly qualified and motivated staff in the NHS there are also a large number that are over promoted, under trained and  under resourced

    Today I have witnessed Physicians associates running entire CWT clinics with no direct medical cover which led to deffered invesgations and repeat visits. The root cause being poor admin and booking of clinics as a result of insufficient admin cover and lack of medical time to triage.

    It is these domino effects that are so prevalent in secondary care that lead to huge inefficiency and waste.

    Delayed investigations or inappropriate falsely reassuring tests because of lack of access to high value diagnostics and expertise is a daily frustration meaning patients languish on wards adding to the spiral of cancellations and delays.

    Cysts are almost universal they appear so commonly on scan. Unless pretty big, ie satsuma plus nobody gets too excited.

    Varicocele is a potential cause of testicular ache, more commonly on the left.

    Assume they are not concerned about epidiymo-orchitis if no response to antibiotics.

    Currently going through this. All work PCs use a webmail link. I work in a lead lined department with a number of Faraday cages. No mobile signal. 15000 employees and not enough money for paper roll for the couch. Work phone lol.

    Earlier on in my career I was involved in many a defecating proctogram(look it up!).

    We used to mix up the barium paste with smash instant mash before it was inserted into the rectum and then pooped back out👍

    My username might give you all a clue to how I pay the bills so I would add to the above:

    Huge number of benign causes of lumps on the testicle itself.

    Cysts of the tunica, appendix testis, scrotal pearls etc are the most common.

    What you think is definitely testicle may be epididymis etc.

    So. 1.GP. 2. Scan. 3. 99 percent of the time nothing to worry about.

    Another by product of these works is that I no longer feel safe crossing the roundabout adjacent to commute to work. They have closed off the North bound cycle lanes for the works and made it pretty dicey getting to work. Bus it is.

    I have nothing useful to add other than this post inspired a root about in the loft. Thankfully haven’t thrown out my entirely unused 6 ply maple Santa Cruz Rob Roskopp from the mid 90s 😂

    Roubaix pro tubeless. Lively, decent puncture protection. Used them for a year or so and roll beautifully at 50 pis with my 90kg weight

    I put 4 falken euro all seasons on our kadjar as they looked like the cross climate and were cheaper😂. The main reason was muddy cyclo cross event car parks. That never materialised but have been happy enough with their winter performance

    It’s interesting that your GP has sent you for the MRI as well. An ultrasound would be easier to get done and would diagnose a tenosynovitis there and then. Fingers crossed for a good outcome and sorry if I sounded doom and gloom!

    I would be interested by what the physio said thinking it was maybe a bone injury. They are good at diagnosing soft tissue injuries and something must have caused them to think bone. I would be concerned about an occult scaphoid fracture and subsequent avascular necrosis. The MRI may help with this.

    I ordered a dengfu time trial bike via ebay.
    It arrived after 3 weeks damaged despite being packed well and the modular fork was too short for the headstock.

    Had the frame repaired at my cost to reduce turn around and used a standard fork whilst waiting for a replacement, which took 3 months.

    Rode it 50 miles and then sold it at about 10 percent of the total cost.

    Just depressed myself rembering the whole situation 😂

    What is the point of halting a test when you hit a number that is essentially picked out of thin air?

    A ramp Test is a test which terminates at failure to complete the last ramp.

    Assuming it is power based your heart rate shouldn’t be a factor.

    If you have a 20 minute climb you can do the classic 2x 20 sub threshold/ sweet spot session.

    Climb 20 minutes around high z3 heart rate. Roll back down and repeat.

    Google Stener lesion / ucl tear.

    This is a potential consequence of the injury you describe but difficult to tell from the history alone.

    The trauma clinic should be able to sort things out.

    Yeah, boot is a little small, though that was compared to the boot on our megane 2 estate, which was massive.

    Frozen shoulder is a diagnosis of exclusion and your history does not seem typical. Seeing a physio especially an extended scope one seems the best bet. A scan to rule out a cuff tear first would be best then it maybe you need treatment for impingement etc.

    Try the CAGE questionnaire.

    A good place to start for many and an indicator of how easy it can be to be blind to problem drinking.

    The casual attitude to excess drinking exhibited by a lot of otherwise sensible people astounds me.

    Coming from a less than salubrious town in the North west to the North East via Oxford I have found a few things to keep me going!

    I now live a few miles from the Beach, a short trip to the Toon and a couple of miles from some decent trails and an extensive network of quiet disused railway lines and roads of the north Pennines.

    Houses are cheap as well.

    Radiology Department in a big acute trust with a high consequence disease unit.

    Not sure what is around the corner for us.

    Definately an uncertainty about my role and whether I may be required to extend into other areas of imaging to help cover.

    My main concern is losing colleagues to self isolation etc and trying to keep up with the non Covid side of business.

    Look up anterior knee pain and prepare to be depressed.

    It sounds like you have had an assement from a physio but not been back again?

    If it is a tendinopathy,those who have already posted are correct and it can be months rather than weeks.

    I had agrivation with my quads tendon at the end of 2017 when I took up Mountain biking again as a change from the road.

    A year later I am back to cycling regularly after a combination of rest, Quad strengthening excersises, massage and Ultrasound treatment
