    Potential minefield. As many have said can do harm as well as good.

    Other side is mortgages, life insurance etc – if screening becomes widespread they may ask to see results before agreeing. Same with health insurance.

    The psychological impact on the individual is also potentially huge – that and many current medical treatments are to treat NOT prevent disease. No point finding something if you still have to wait anyway. Well there is for a symptomatic conditions.

    Whole body CT has downsides – some argument to stop using in trauma unless indicated (from Germany)

    I have hydraulic shimano discs on my road bike now and I would never go back.

    Difference is like night and day vs rim brakes.

    Can stop very very quickly and without locking up.

    Private is better for speed and if you want to see Dr x and only Dr x each time. For day case or overnight stay surgery it is good.

    Surgery which is anything more there is an argument to support choosing nhs.

    As per posts above limiting factor in NHS is capacity. Yes consultants could work more hours but there is also the issue of paying for the extra work at a time when NHS funds are not exactly flowing!

    The issue of emily more doctors is workforce modelling is difficult – leaving medical school to becoming an Orthopaedic surgeon as an example – absolute minimum of ten years. So you are always trying to guess how many doctors in x,y or z will be needed in many years to come.

    As pointed out above you can choose to be seen at a private hospital through NHS treatment if it’s not emergency stuff.

    As per andyfla – there was a target that so many people had to be able to get short notice appointments.

    Only way to do this is not allow many to be booked so people can drop in on day.

    Short answer: ALL parties use the NHS as a political point scoring tool. Until this stops things won’t improve hugely. There are difficult decision ahead in some ways re cost of healthcare and what is provided and not. There are also efficienc savings to be made in many areas with the right approach.

    Most NHS staff try their very best.

    Bit of feedback from today’s ride – cant fault the 4. Managed 62 miles all up at 18.5 ave on a bunch ride. Some were out on carbon best bikes still and on the short sharp climbs when everyone was smashing it it was hard to hang on. Rest of the time was fine though.

    Bike very comfortable running 28mm tyres and massive amounts of clearance under the guards (fitted P45 chromoplastics as they also clear 35 cross tyres for off-road commute).

    In short can’t fault it – the 11 speed is good and the hydros are awesome. The main benefit (to me) is they are totally consistent and predictable and you can scrub speed quickly and easily with one finger on each lever.

    It works well as a road or off road bike – clearly it isn’t perfect at both as there have to be compromises but it is a very good all rounder.

    Can’t comment on the size small as mine is an xl!

    Road wIse even with 35mm cross tyres on it’s ok – bout 1mph or less slower than the good bike on the commute.

    Doing a club run on it in the am which will average 20mph ish so will report back…

    The flare drop bars are surprisingly good too on the road, not sure why though!

    Also got a 4 – done about 100 miles on road commuting and also 50 mile ish mix road and off road in lakes last weekend.

    Hydros are absolutely awesome and 11 speed is good too – for long off road rides the 32 tooth sprocket is useful for spinning up steep stuff. Uphill was no slower than most mtbs last weekend even in the rocky stuff. Downhill but slower but part of that was running small block 8s at 70psi. Clearly not as fast as an MTB though.

    Not got full guards and 28mm contis on for he commute.

    Cannot fault the bike at all so far – it is a great all rounder and good for a bit of anything really. Could easily do a 100 miler on road and equally reckon it could manage a trail centre red with wheels on ground. Did I mention the hydros are awesome?!

    As above. Very neatly done it is too.

    Just been out on my new 4 today. Guard going on next week.

    4 is worth a consider – the hydro discs are absolutely awesome. Single finger braking even in the wet and very controlled. I can see for winter commuting an idiot avoidance in Leeds they are going to e rather useful!

    Don’t know how you do it but step one is make the NHS cross party – I..e not run by governing party alone – many things are done to win votes and little else,

    Step two – we need to decide as a society what it should cover, eveything for everyone for free is not sustainable. Some hard choices need to be made.

    The NHS does many great things, it also gets some wrong. That is real life and sadly always will be.

    There was a recent survey on whether people would rather pay more tax or see nhs services streamlined. The majority said streamline the nhs, not raise taxes. Interesting.

    Having cross party control of the nhs so it didn’t get reorganised every 5-10 years with each new government ‘improving’ it would be a huge start.

    The fact that this vote happened is excellent.

    Ignoring what each party may have done in the past they seem to be doing ok at present. There’s pm doubt they have been in government at a difficult time.

    Are things perfect? No. Improving? Maybe.

    All of this has at least made me think that if something in society/life isn’t right then perhaps I ought to start by doing something myself rather than moaning about everyone else. If we all did one positive thing each day to help someone else the world would start to change maybe.

    Junkyard your points are valid, ish. Pay does go up but not year on year.

    You also pay thousands a year in course fees, professional indemnity, professional subscriptions etc.

    Yes doctors have a good life compared to many. But if you had got AAA at school and could do pretty much anything at uni would you choose to do medicine if you had to self fund?

    Fees are not paid BTW.

    To give the other side consultant pay is 75k. But that may take 12 years plus to reach AFTER medical school. (much less in some areas of medicine but affected by choice of specialty)

    Aracer – basic pay after 9 years is 45k. You do get a multiplier to this based on total hours and weekends/nights etc but it will still take years to pay off the debt.

    It is likely to totally skew the dynamic of medical school applicants.

    Two observations here – what is ‘brightness’? Success in IQ tests? A 1st in a degree? An ability to reach evidenced opinion in complex sociopolitical issues? You could go on….

    The second – the state vs private. As a medic this is interesting…

    medicine – 5/6 years at 9k fees plus living expenses = 60-70k of debt at graduation or more (unless parents can pay). Starting salary 20k.

    How many doctors are we going to get from state schools/poor backgrounds. It is going to be a huge issue.

    Third observation – what is the role of university anyway? Do we need them and if so to serve what role in education?

    The thing I struggle with in all of this is if the vote is yes a bunch of politicians will still make all the rules but sit in Scotland and not London. In time is their behaviour really going to be that different to any other politicians – will things really be that different in the end?

    Swedishchef – hadn’t swum much since I was 10. Used to run a bit but never seriously, cycling wise did quite a bit but mainly just socially.

    There were 20 bloody hard weeks!!

    If bike not too hilly consider an Tri bike set up – ie aero bars but not too aggressive. As others have said practice bike run transition. A lot! It’s very weird.

    Main advice would be decide how much time you can realistically spare to train and then get a programme built around that.

    Although boring time on turbo and treadmill doing intervals gives v high quality training. Split long runs can help too.

    Programme to work to helps – it is hard enough without having to think about every session.

    It is very doable – I went from having meet done a Tri to doing a 10hr 40 ironman in 20 weeks. At the time I was working 60hr weeks. The key was v careful planning. Am not willy waving!

    The short answer to this is the evidence is mixed on whether you should fix any clavicles – and if you are what the criteria for this should be.

    There is far from uniform agreement amongst orthopods on how to treat.

    To give another side of the argument – surgery does not have a 100% success rate, and things can rarely go far from well.

    When you heading out? I live in Penistone 5 mins up the road and ride round that area.

    Langsett the obvious one or drive out to Edale and do the Jacobs ladder loop.

    There is a good local loop taking in some nice scenery up the old wagon road from oxspring then down to silkstone from the travellers and then via cawthorne to gunthwaite then to inbirchworth and thurlstone.
