    Anyway no point on a daft argument, ironic for here.

    The NHS is and remains an excellent system. Moving to a part of full insurance based scheme would be a disaster. There are few US based medics who rate their systems except the ones earning millions of dollars a year from it.

    Fair point but most of the Scandinavian countries mentioned spend far more than we do on healthcare.

    Part of the outcomes is also patients choosing to ignore symptoms/not present – so cultural as well as economical. Screening for bowel cancer was not taken up by a lot of patients in UK trials.

    Commonwealth fund have some nice data on this – we spend some of the lowest amounts per head in the developed world on healthcare. And overall get some of the best outcomes.

    Michelin mud 2? are excellent in the slop and don’t roll too slow either. And inexpensive.

    Ridden them in south York’s winter slop with good traction.

    Ninfan not that simple. If you work part time you still do nights weekends etc just not as often.

    Means many areas which have lots of antisocial hours working (eg A/E which canbe two weekends in three) struggle to recruit and then to retain.

    Interesting to read opinions. There is no doubt morale in the medical profession is falling off a cliff.

    The new contract is probably a.push toward seven day working which is a huge issue – as mentioned elsewhere 7 day working is just a redistribution of staff from 5 days to 7 = lower staffing levels.

    All the pies – cut is to pre tax salary. Will vary post by post due to complexities of pay system but for most will be 10-30% cut.

    To put that in perspective due to changes in hours etc I got better pay in first year post medschool in 2002 than our juniors do now 13 years later. I worked crazy unsafe hours so some things have improved and pay is still better than many being fair.

    To answer some of the above – no one is asking to work less hours. Most contracts are 48 hours a week – European maximum. Many European countries medics do 35-40.

    The government want to remove financial penalties where docs are made to routinely work over 48 hours average.

    No one is asking for less hours, just not a pay cut.

    Many rotas already have up to 50% empty slots as no one is applying for jobs any more. The proposed changes will make this far worse.

    Medics give advice as per what they feel is best recovery. Not always evidence based as often there is no good evidence.

    As I say to all patients, ‘I am here to give aadvice, you choose whether to liste to it’

    Down to the the individual to decide priorities

    If you want a job in a competitive specialty you need to present at international meetings. It is common to have to self fund this.

    Professional subscriptions and indemnity are 2k and up a year and most courses, many of which will be self funded 800 quid and up.

    Other professions prob have similar but wih six years of fees, reduced pension, proposed pay cuts, ever increasing work intensity medicine is going to struggle to attract the best before long.

    Most day to day ward work is done by ‘juniors’. As are many clinics and operating lists.

    Still a good career and we are lucky but pay is awful. I got paid a better salary in 2002 first year post grad than out juniors do now. Admittedly I also worked 75hours/week for it.

    Maninthenorth you are right to a point. Medicine by definition is making decisions often based on only a certain amount of information which may have massive consequences if you get it wrong.

    That needs a certain amount of self belief. Fine line that though.

    The contract changes plus pension changes plus med school fees are a disaster though. More and more of those leaving medical school are going overseas or leaving. Some rotas are now 50% vacant and getting worse….

    Mate has one and has done lots of m way miles with his carbon road bike on roof.

    Not had any issues. Seem bombprooof. And very clever.

    Morzine another option – short transfer, lots of accommodation options. Joux plane straight from town. Ramaz, joux vert, colombierre, various other climbs all nearby. Low too so less chance of snow

    If you call the clinic and say you’re having problems and want to be seen sooner you should be. We certainly do.

    The evidence for fixing clavicle fractures is far from concrete except in very specific fractures in specific patients.

    EVOC bike bag works well – taken my solaris and carbon road bike on multiple trips overseas (seperately!). Always worked well.

    BA will take bike bag as part of normal luggage too if ok weight wise (I got away with 30kg this time as put all my stuff for a week in there – that might be chancing it though!)

    Not operating for swelling is right thing to do – get that wrong and it can go very very wrong.

    Weekend trauma lists are life or limb and also for various reasons little old deals with broken hips get prioritised over young healthy folk. That’s a national level decision not a local one.

    From the info given it sounds like the right things are being done and it is all sensible. The communication over why sounds like could be better – may be reasons why but who knows.

    Not seeing consultant every day is standard – they don’t do rounds every day.

    Hope you get fixed soon. If it helps a fracture fixed at ten days well so far better than one fixed at one day badly with swollen tissues. Doesn’t relieve the frustration and boredom though.

    Have a road bike with discs. Have a roa bike without.

    Would get discs if an option. Consistent all weather braking and no rim wear on wheels either.

    Also huge control over the power with a single finger.

    But heavier than rim brakes but unless you’re 5% body fat bit irrelevant??

    The doping is almost irrelevant. As others have said – it’s the way he tried to actively destroy the career of anyone, cy link or non who stood in his way. And at any cost. Not on.

    That and the fact that he acknowledge he has no regrets and would do the same again. Which implies he sees no need for remorse over his actions.

    It is hard to conclude he’s an alright bloke!

    To present another side – launching an investigation is just that. They may have no intent of doing anything but have launched it to be seen to be doing something to manage public expectation.

    That may also be bobbins but until anything/anyone is prosecuted launching an investigation is just that.

    As has been said before and less relevant here – if govts want to stop tax avoidance change the law to make it impossible. They could do it if they chose yet they don’t.

    We have the most or one of the most coat effective systems in the world.

    Issue is rising demand. Yes it has inefficiencies. Many other systems have greater ones.

    There is a big debate on the horizon – spend more or ration more.

    Lot of valid points. Community resources huge issue.

    Saw a patient the other day in clinic. Not managing at home due to injury. Required no inpatient medical care.

    Social services involved – no capacity.

    Solution – admit to hospital despite needing no medical care.

    Isolated example but like using a missile to crack a walnut!

    Offset – as above. See if you can switch to this if an option.

    Best of both worlds – pay no interest on mortgage so you can either overpay or reduce monthly payments and you still have accessible cash.

    Gives an effective savings rate way higher than otherwise available even with long lock ins (if a taxpayer), and as commented above mortgages getting far harder to get – so if you change your mind down the line could be tricky if paid off.

    Mortgage borrowing likely cheapest borrowing you will find…

    Interesting thread. Can’t decide on S & M (bars!).

    6’4 and Sheffield based so hilly.

    Any advice welcome – going to try demo both before I commit.

    And agree – raw lacquer is awesome. I understood part of premium is they only use very best frames for them because they are less forgiving with no paint. This may be horse manure!

    Would get it looked at – as per others may be more going on than you think. Unlikely but important to check – certain missed foot injuries can be a long term problem
