    Hi Becky

    I am an orthopaedic reg – as mentioned above the answers vary depending on which path you go down.

    In short it’s a great job, no two days are the same and it is highly rewarding and enjoyable. There is a lot of crap to put up with, and especially in hospital medicine the career ladder is very hierarchical if that makes sense.

    The guys leaving med school now have a tough time – the money is lousy due to the hours cuts – I got more 10 years ago than they get now.

    Realistically if you want to do hospital medicine of some sort you are looking at 10+ years of hard work post med school assuming you work full time (if part time double it) to train to consultant level with exams along the way. It is at times very hard.

    No amount of money in the world would make me change career – that probably tells you all you need to know. If you can find the right branch of medicine for you (and the spectrum is huge) it is worth every minute.

    As per above – treatment costs going up and up. Putting a 700 quid plate on a wrist not two forty pound wires uses up money. Not that hard to put 10-20k of metalwork in a spine now.

    Add that up across all specialities and all treatments and costs are going crazy.

    Equally 18 week target means yes everyone is treated quickly but far more are being treated too – even more costs, especially when lots of surgery being done in private sector/outside hours to meet targets.

    It may have changed as was there a while ago.

    Having worked in Oz and UK as a hospital doctor over there they would have 7-8 nurses for the same number of patients that over here we would have three nurses.

    As has been mentioned above the proliferation of specialist nurses has provided an excellent service. The downside is they are highly expensive – often per hour more than reasonably skilled junior doctors (i.e. anything below consultant). At the end of the day the more money you spend in one thing the more you have to save elsewhere – i.e. on ward staff.

    The message very much seems to be that ‘just’ being a ward nurse is not good enough and unless you are in a specialist or management post you have not done well. It would be better if some of the very senior nursing staff could be kept on the wards.

    disectomy surgery for LEG pain gets very good results. For BACK pain the water is a muddied to say the least.

    Have seen many patients who have gone and done pilates/yoga and come back saying it makes a big difference. No science to back it up but may be worth a go….

    (especially as cyclists CAN get tight hamstrings which can cause lower back issues at times

    IF it is a disc (could be spasm alone) high chance will resolve on its own +/- physio within 2-3 months.

    If not settling at that stage consider GP referral to neuro/ortho spinal surgeon.

    Physio/staying active once easier will help, especially core work etc.

    Lots of back pain/’sciatica’ sufferers find pilates very helpful for what it’s worth…

    Most likely rotator cuff minor tear or arthropathy +/- impingement +/- adhesive capsulitis.

    Treatment in first instance physio. ULtrasound +/- injection if not settling.

    ‘broken hip’ covers quite a few injuries. It is probably (if being plated) an extracapsular fracture which is the better option if you are young (less risk of problems down line).

    In time he should recover and do well, but if it is this fracture type it will collapse as part of the normal healing process to a certain degree. In some (far from all) people this will leave them with weak hip abductors and possibly walking less well than previously. Equally many patients of his age group in time make a full recovery, but bargain on three months plus before normal again.

    Despite the rubbish in the press it can be a straightforward ish procedure with low complication rates but this is dependant on the fracture pattern – there is a broad spectrum from stable 2 part fractures to highly unstable 4 part plus fractures.

    Hmm will give it a go. Is it a guard’s van job or just jump in a carriage? Sorry for all the questions! Trying to avoid using the car and train will make a 60 mile a day commute more like low 30’s by bike.

    Thanks for the ideas. Will shout up nearer the time and see if anyone is riding. If it’s flatish something near 90-100 miles would be good.

    Buzzrunner 3. Fantastic rack. Locks to towbar, holds three bikes, adjustable wheel slots, all bikes lock to rack and lightboard etc included.

    Also has a good tilting mechanism and VERY easy to get in boot even with three bikes on.

    Not much info around about them but got one and has been to south wales and back twice and to the pyrenees and back from yorkshire with no problems at all.

    For the money it’s a cracking buy.

    Sure in other weather could have been a fun day. The wind and rain were bloody miserable though.

    On the plus side after some impromptu off roading on one of the descents have discovered mavic ksyriums are pretty bombproof!

    Simple answer is you need an MRI – which is now being done.

    In an ideal world it could have been done quicker but the flipside of that is if everyone who had a painful swollen knee went straight to MRI the wait would be massive.

    Clinical judgement is necessary – and that is what it is – an opinion – which can sometimes be wrong.

    It is highly unlikely much further damage done, although not 100% impossible.

    MRI should give a diagnosis and a good idea of what needed for improvement.

    Although they are not the lightest they perform well. And unless you weight 60kg does an extra pound or two on a bike matter?!

    Haven’t quite got that far yet.

    Guess the ksyrium sr’s will be going on it.

    And probably DA as all other road bikes are shimano. Arguably a crime but pref not to run mix of campag and shimano across bikes, and heard mixed reports on SRAM stuff.

    The rest am open to suggestion!

    Ah the joys of cut gate. Never ridden right down to fairholmes – usually go out from langsett then turn back at the ridge/cairn.

    Is it worth the drop down to ladybower and back up?

    And yes the wagon and horses is a cracker, as is the bank view cafe with doorstep slices of cake

    Go see a solicitor. Not that bad cost wise and it’s their area of expertise.

    In short she can say anything, you can deny it and still get divorced.

    Can recommend south yorks – west of m1 near barnsley.

    Lots of villages, easy to access to Leeds/Sheffield/manchester if needed.

    Fantastic mountain biking and road biking – peaks are 15-20 min cycle. Cut gate is an easy ride from the front door…

    Decent folk too.
