    FYI it is extremely extremely rare (pretty much never) that it is necessary to operate on a clavicle fracture in children, and far from common even in early teens.

    Would worry not to be honest – kids are very good at making bent bones straight again…

    The realistic chances of finding anything that significant in an asymptomatic individual is low – hence not offered on NHS.

    You also have to consider the potential morbidity of the investigations.

    In short if you are well not worth worrying about unless very strong family history of anything unpleasant (i.e. first degree relatives etc)

    OP would think you would want shorter not longer in TT? If scoliosis correction presume fusion is at least upper thoracic down to L2 ish?

    If minimally displaced it will be sore for two to three weeks. Assuming this and it being a stable injury not a big deal. Lots of painkillers and get the elbow going when you can – will stiffen quickly

    TurnerGuy – shifts work ok in somewhere like A/E. Having done 12hr shifts and 24hr call with usually 5-6 hrs sleep onsite I would rather work 24 hrs – it seems daft but less handovers, more continuity and less antisocial to work too.

    Could be nerve root compression secondary to a disc or other causes, brachial neuritis maybe, radial nerve compression although unlikely along with many other things.

    Diazepam may be worth a try if you can get some prescribed.

    Love mine is an xl with 50 stem and haven bars – not sure on width but wide.

    Feels perfect and am 6’4. Feels way less perched than any previous mtb and much more like a bike that fits.

    Went hope hoops crest, now arch ex tubeless crossmark and ardent, white reba, XT 2×10 and Easton haven carbon bar/seatpost and haven stem.

    Awesome bike, prefer to fuel ex it replaced.

    If you are in boot and weightbearing it is by definition a stable fracture. If they are happy for you to do so doing range of motion exercises out of boot when seated will help hugely.

    If you have a turbo you can get axles to put pedals outboard so you can pedal in an aircast boot – have had patients do this previously.

    Long term should be fine and shouldn’t be that sore when you get back to riding. That said everyone recovers differently and it is a bit of a suck it and see.

    As above – achieve injury covers a massive spectrum of injuries. Simple answer is when it hurts less try riding – you’ll soon discover if you can ride ok or not.

    Apart from elite athletes/manual workers and more severe injuries very few of these have to be fixed. Some justify discussion of fixation.

    Something I struggle with – there are many BTL landlords around who bought lots of cheap properties and rent them out.

    One day these landlords will need to sell them all and realise capital.

    Who are they going to sell them to if mortgages stay as they (rightly) are now and require 20%+ deposits?

    Am I missing something??

    Try decide what you want in advance.

    The phone call of ‘I want car x in this colour and model what’s your best price’ worked well last time – got a big discount off the price just over the phone and more when went in.

    No published evidence that parachutes save lives either…

    I have now caved in two helmets. Not my head. Apart from cost which is small I cannot see a reason not to wear one.

    That said it should be a choice.

    If clipped by a car and sustaining blunt head trauma on Tarmac I’d rather crush helmet than skull

    Many of these figures are pulled out of the air.

    As previously mentioned the pension reform has much to do with nhs privatisation.

    If civil service and especially mp’s reform their pension to pay more and work to 68 they can change mine too.

    The image of rich golf playing medics who are never at work is somewhat outdated. That said it is far from a bad lot but an easy life it is not.

    One point which was referred to earlier on – it has been on the record previously that a major stumbling block to privatising the nhs is doctors pensions.

    A key aim of all this is to enable more privatisation of the nhs. This is in no way as silmple a problem as it seems.

    Interestingly of I had children applying for university right now who could get into medical school I think I would tell them to do something else which seems a great shame. I would say the same regardless of whether the pension changed or not.

    Jambalaya sorry but wrong. Nurses follow defined pathways, doctors do not. Some things can be done by nurses, but suggesting gp’s are not needed is farcical.

    They get a lot of stick and as pointed out above some do very well. It’s far from an easy job on that the consequences of getting it wrong are often massive.

    To add another side – pension same as mp’s etc. only they pay 50% less in contributions to the proposed deal for medics.

    If they change theirs they can change the medics.

    And yes we live longer but will my operating skills by that good at 68? I am not so sure on that one….

    Can be fixed but anterior dislocations usually left. Posterior ones are stabilised as posterior ones can have serious complications.

    Surgery carries some small but fairly monumental complications if things go wrong – there’s a whole lot of pulsatile things not far from there….

    90% resolve in 8-12 weeks with painkillers and physio.

    Beyond that less chance of simple resolution – options beyond that if it’s a disc are injection or surgery. Surgery has good Outcomes in most but not guaranteed obviously.

    Read an interesting article – he’s been doing huge amounts of training apparently with aim of peaking for the tour and the dauphine is just being used as a run out.

    He looks strong, but guess it’s all down to the hills.

    Having thought about this for five seconds, there are folk I work with who spend upwards of 50k on a car which is madness.
