    A&E don’t prioritise!

    Bet you we do, but only into categories….

    Burnt fingers and sore wrist are both Cat 4’s so whichever of the Cat 4s got there first get’s seen first.

    SOB (Short of breath) is probably a Cat 3 (depending ion how bad you are at triage) so gets seen before Cat 4.

    Although I have to ask, if you’d seen your GP and he thought it was bad enough to have you admitted why did he not have you admitted, why did he send you to A&E rather than the admissions unit? Bloody GPs that do this really boil my piss, just creates extra work for me, not your fault but it allows me to have a rant. (Maybe if he’d sent you to the right place you wouldn’t have waited 5 hours – although given how shit our local admissions unit is, it would have been closer to 8 so think yourself lucky)

    Our working theory is that they have a load of GP mates who convince people who show up with ‘Embarrassing Illnesses’ to appear on the show with a promise of immediate treatment from a top Harley Street surgeon/fanny mechanic. (they never seem to get their treatment at their local hospital…)

    I’m looking for the same sort of car and have been eyeing up the VW passat estate…. Slightly bigger load space than the octavia and some/most seem to be cheaper strangely (presumably because everyone wants an Octavia?).

    Tazzy, that reminds me of a brilliant idea we had for making money for the NHS:

    Turning half of our reception desk into a kebab outlet…. all the drunk people would love a kebab while they were waiting to be seen on a friday/saturday/sunday night, it’d make a killing.

    (We also thought, to save on outlay, you could make the elephants leg from all the offcuts from theatre thus meaning minimal outlay on raw ingredients!)

    I don’t mind the people who have a few drinks and then fall over, trip up or otherwise injure themselves, they tend to be reasonably pleasant and usually apologetic for “wasting my time” (which they aren’t). The two groups of patients that tend to get on my wick are the ones who are in A&E for no other reason than they are drunk (a fair number of people on a Thursday – Sunday night) and just need a place to lie down/be sick/spit all over/shit themselves in while we keep an eye on them until they sober up and the group who go out, get drunk, have a fight and then come to A&E and give the staff grief.

    For the first group: I am not your mother/best mate. This is their job, not mine.

    For the second group: Don’t blame me coz you are a scrote who goes out to get drunk and have a fight and foolishly chooses the same night as everyone else, if you do this it WILL be busy and you WILL have to wait. And no, “paying my wages” does not give you the right to be seen immediately or swear at me.

    Charge the first group, arrest the second. Simple.

    Gonna update my Dell Mini 9 to 10.6.6 this weekend (just for something to do), it’s been running 10.5 for yonks now and is lovely. Much smaller footprint than windoze, iWork instead of office, iTunes and Firefox.

    Ventilation is very very rarely (never?) the limiting factor to continued exercise….

    I looked at these a few years back and the only papers I could find written about them concluded that if your lungs were a bit shit to start off with (i.e. you are a 70-odd year old granny with COPD) then they will give you a bit of benefit but if you are young and fit and healthy then they do bugger all. Willing to be proved wrong though if there has been more recent research.

    They will always be referred to fracture clinic in that situation though where an expert will view the film to check the AE doctor got it right

    Radiologists, like most NHS Docs, have timetables. Sometimes they are reporting X-rays, sometimes they are performing clever scans and sometimes they are feeding ridiculously expensive platinum coils into peoples heads. Having one just hanging around awaiting to look at every x-ray as it uploads to the server would be even more inefficient and require more radiologists than there are….

    Probably just secondary to inflammation but you need to pop along and see your doc, make sure you’re not brewing something nastier.

    (Chocolate [well, theobromine in chocolate] has been shown to be a better cough suppressant than over the counter cough medicines)

    I’m working in A&E and I think I’ve only seen about three people who genuinely have had the flu….

    I have however seen about three million people with a **** cold and no testicular fortitude. I have been prescribing a lot of MTFU – Stat.

    BR, that report is from twelve years ago and only gives the cost to the NHS (also massively underestimates it IMHO). Doesn’t mention how much the other problems cost, e.g. ‘About 50 million working days are lost as a result of smoking-related illnesses’ and things like carers cost.

    I reckon its about cost neutral at best.

    Oh yeah, and either way you’ll be off the bike for 6-9 months depending on a variety of factors. Having it ‘repaired’ doesn’t actually fix it, it just stitches the broken ends next to each other so your body can fix it. This explains the decreased re-rupture rate as they are more closely approximated. However with operative and non-operative treatment you still have to wait for your body to actually fix the rupture.

    Me too, very little advantage of surgical repair for your average punter as far as I remember….

    According to Kahn et al, there was a consistent finding of an approximately 33% higher rate of complications (other than rerupture) in those treated surgically1 ; nonoperatively treated patients had a rerupture rate approximately 3 times higher than those treated surgically, but these patients had minimal risk for other complications. Listed complications resulting from open surgical repair included deep infections (1%), fistulae (3%), necrosis of the skin or tendon (2%), rerupture (2%), and minor complications (percentage not documented).

    Might indicate a DVT but studies show its a bobbins test and if there is a clot is likely to break bits off and send them upstream.

    A rule of thumb for working out the weight of a child is: (age + 4) x 2 = weight in kg.

    (and with regard to the pacemakers thing… they use large magnets to turn them on and off too so putting yourself into an MRI scanner will turn your pacemaker off – not necessarily a good thing)

    As a rule of thumb the percentage likelihood of dying from your burns is the percentage of body surface area burnt + your age.

    (Not including first degree burns)
