    Your point around replacing GP’s with other professionals and the associated costs when they have to refer on when they reach the limits of their speciality is interesting.

    IME in prehospital care since specialist centres have been introduced (PPCI, Stroke and Trauma Centres) the evidence suggest clinical outcomes have improved for those patient groups. Even though it might mean a 45min journey to the specialist centre instead of 5mins to nearest ED. I had imagined that this model would translate to primary.

    With regard to the costs associated with referrals from non-GP healthcare professionals, how does this differ to a referral from a GP to a specialist service. For example – I have a musculoskeletal problem, I see a GP who prescribes some pain relief and then refers me to a physio vs I see a physio at the GP surgery who gives exercises and treatment and then refers me to a GP if they are unable to prescribe? Clinical outcome is broadly the same as a measure, perhaps better as the physio treatment has started earlier.

    Maybe I’m looking at this from an over simplistic point of view, based on my observations and experience. Maybe clinicians in primary, secondary and prehospital all feel they have a better understanding of how things work. I haven’t a clue. I’d be interested in learning more to help me understand better.

    I certainly agree that some roles and functions cannot be replaced with other healthcare professionals with limited scopes of practice, but I think there is a place for some, such as physio’s, in primary care.

    Try a tablespoon of oil in the dough, it helps soften the crumb. Its what iuse in my bread recipe, I don’t use a milk wash.

    Temp wise, I start high and then lower once the crust starts to go. I can’t remember the temps I use off the top of my head, it’s been a while.

    My experience,

    I use pfSense on my router, any VPN is set up on the router. Not at straight forward as logging in, you need to get the setup information from the VPN provider for the server you wish to use in the country you want. Once set up you then need to route internet traffic through the VPN tunnel, this can be all traffic or from specific local IP addresses.

    I can comment on routers provided by your ISP or 3rd party routers from the likes of ASUS.

    Sounds like a couple of possibilities, I’m making an assumption that you’ve not checked these as you’ve not mentioned them.

    The mech is out of alignment and needs re-indexing. There is usually a collar on the shifter, mech or both. Have a look at the alignment and adujust it until the clicking stops.

    You say you’re about to change the cassette and chain. Is this due to wear? How long since the last cassette and chain change? As the chain wears it can jump, this is worse if the cassette is due a change.

    Best definition I’ve heard of ultra processed food is if items on the ingredients list are things you wouldn’t find in your cupboards at home then they’re ultra processed.

    I wouldn’t, circular saws are vicious when they bite and kick back!

    Bow saw for green wood is best hand tool for the job. Best power tool for the job is probably a chainsaw (just as vicious at times).

    What size tree?

    If you’re taking the smaller branches off a fallen tree, take care that it doesn’t stand up again as you take the weight off the top.

    My daughter enjoys Hogwarts Legacy. Some exploring, some puzzle solving and some battles. No swearing as I recall. Not online though, but that suits me.

    Not sure it really counts as FPS mind, you shoot with a wand.

    Which area are you in? I’m in York with CityFibre and have a big choice. We went with No One internet. Service has been good, customer service has been very good. Gave me a hand in setting up my own router, gave me all the info I needed to get it all up including IPv6.

    Pavement Vaults, Ambiente Tapas (although that can get pricey quickly) amd the Chopping Block are some of our favorites. But you’ll be spoiled for choice really.

    If I remember correctly if you make a compass the needle points back to your last set spawn point (wherever the last bed you slept in is).

    Edit: just seen your post about sleeping in a bed.

    When this happened to us we built a new settlement and noted the coordinates.

    Depends what you define as cheap. I have some Denon ANC from Richer sounds.

    They seem to do the job for me. Paid around £80.

    Sorry, trying to keep posts as short as possible, I ‘ve been with TalkTalk for about 6 years, current contract is 24 months.

    I suspect that the later email is automated and can be ignored. They’d have a hard time justifying a charge when you’re jumping ship because they’re terminating the service, and are breaking the contract early themselves. You should be charging them. 😁

    I put this in the last email I sent to them. Still waiting to hear a response.

    Presumably this “obsolete” network won’t be required to deliver the new service. So don’t cancel it, just leave it. There’s no harm in having both services live for three days, and you’re guaranteed continuation of service on switchover day. It’s what I did when I went from Virgin cable to an FTTP provider.

    Actually the fibre cable entering the property is a CityFibre cable that can be used by a number of providers, including TalkTalk. I am somwhat confused as to what is becoming obsolete and means they are unable to continue providing the current service. Especially as if I enter a new contract, as is one option, with them the service will be delivered via the same cable. The new provider will also use the same cable to the property.

    @Cletus – yep. About 6 years ago full fibre (1Gb) was installed on a trial basis. TalkTalk have now decided that the infrastructure is obsolete so they are turning off the network. I can however decide to stay with them and pay £10 more a month for the same service, through the same fibre cable. Or in their words leave for a new supplier with no charges. And here we are with charges being applied and unable to talk to someone who know what’s going on.

    @mattyfez – no worries. They are trying to charge me for early termination of contract, despite the fact they are turning off my service a month earlier than the contract end date (due to the end of a trail in my area) . And I’ve asked to leave 2 days before turn off so that I can maintain an Internet connection.

    – stopping the direct debit is on the to do list as soon as I’m disconnected. They’ve said final bill will be raised 10 days after disconnect.

    @bruneep – Unfortunately other than here and a Google acount I don’t have any social media accounts. I closed that last 2 years ago because it was sucking the life out of me. I also find social media anything but social.

    I wonder if it’s a platform thing? On Android using Firefox with various privacy plugins enabled and very rarely get logged out. Maybe every couple of months. Not enough to notice. Never had an issue with speed either.

    The our world in data website gives a good breakdown of carbon footprint for each mode of travel. Although it only looks at CO2, so while it rates a ferry as a foot passenger as one of the lowest impacts, I’m aware of the other pollutants associated with heavy oils.

    You should be able to use some cloning software that’ll copy your current drive and expand to fill the new.

    Acronis is one that jumps to mind, but there are several.

    Relatively straightforward task for a home user.

    Depending where you’re travelling from in the UK, have a look at Newcastle to Ijmuiden.

    We cross a couple of times a year and chop and change based on price. (Travel from York, so journey time this end similar)

    But yes, prices have been ridiculous since travel restarted after Covid.

    A good locksmith might be able to ‘decode’ the lock for you and make a new key for it.

    That said those locks are really low security. It won’t stop some one for too long from getting into your garage.

    I’ve never understood the prosecutions brought in cases involving vehicles. In my head, based on the information in th op, there is alleged criminal damage, the vehicle has been used as a weapon to assault/attempt assault, in addition to dangerous driving. If the evidence supports the allegation why are drivers not charged with all of the offences committed?

    We had our driveway done about 5 years ago. Within 8 months we had dips forming where the car wheels sat.

    We paid for the additional ground works as this was a requirement for the warranty. Over the period of 2 years tried to get someone out to make good. The company declared bankruptcy, strangely there is another company with the same logo and one director trading now.

    I’d never go with a resin driveway again.

    And weeds do come through despite the sales pitch.

    Edit: we still have a drive with ever growing dips!

    If your printer is still serviceable is there an option to purchase a Raspberry Pi and set up a print server on the local network and print via that?

    I could save you some cash vs a new printer.
