Looking at the Transition website you need a 44 top - 56 bottom

2012 so looks like 44 ID top cup, 49.6 ID lower cup.
Will that hope adapter work with other headsets? I think it already has a superstar fitted.

Yep the adaptor goes on first. Then your crown race goes on the adaptor. Works with any 1.5 lower cup.
