Electron Backupz usb light - had a front/ rear set for 3 years now, most reliable lights I've ever owned.
In decreasing luminosity and cost
Exposure Joystick (rechargeable)
Cateye EL620 (rechargeable)
Cateye Uno (single AA battery)
All sit nicely under the handlebars. For the money, the Uno is an excellent light, but the Joystick was an absolute bargain when Wiggle were disposing of them.
Something that flashes, don't need to be too bright.
I got one of the smart 7's. batteries last ages. one high, and one low if you really want to be seen.

Something like a Joystick is more 'seeing' than 'be seen' - it hasn't got any sideways light spill like front bike lights for 'being seen' have built in. It is almost as much about sideways light as forward light for being seen.
USE Flash (or Flare, whichever is the front one!).

£7 quid. i was impressed. no long term opinions on it though... although my phaart rears are doing great.
+1 for the USE (Exposure) Flash (Flare is the rear). I've got a flash/flare combo on my commuter and they're very good IMO. Not the cheapest mind and I use at least one extra rear light and a helmet mounted joystick (for seeing where I'm going) for the front.

If you get the white ones they actually glow with the light on so you do get some side light visibility (good for the rear red light too).

flash and flare - the pulsing is nice as they are on constantly - flashing has dark spots and could conceivably be missed in a scan.
