Has anyone just inserted the axle, fitted the non driveside crank up to the BB bearing & then tightened without torqing it up?

Torqueing what up exactly?
The BB?
The end cap?
The pinch bolts?

if you mean not fitting the pre-tensioner thingy, err, no. not me anyway.

The end cap, in the Alps last year one guy stripped a pedal thread & fitted a HTII BB with just the cup spanner & allen key - it seemed to work fine without it.
