If its a 68mm bb shell just use a 73mm one with the guide as the spacer.I think you can get ones for E-type front mechs which will work with a guide instead.

I'm using a bb mounted E13 drs with an isis bb so I would say yes. I had to still use the driveside BB spacer though otherwise the granny ring bolts scuff the plate of the guide. Might change the chain line a bit, doesn't really seem to make any difference though.
