I am hopefully off to Nepal in the spring to take on the Annapurna circuit by bike and was wondering if any one could reccomend a travel insurance firm who cover stuff like helicopter medi-evac at over 4000m (the limit for most like Columbus) for silly cyclists.

    All advice greatly appreciated.

Have you checked SnowCard and DogTag? Can't be arsed to look and see what their limits are, but I've used them for alpine biking and Himalayan trekking, so might be possible. If they don't do it as standard then it might be worth ringing to see if they can extend their limits for a fee..
