May be able to help on the bar front... Sunline V1's, 19mm rise, 720mm wide.
And can definitely help on the adapter front, only it's a Shimano adapter, not A2Z. I used it for running a 4pot XT IS mount caliper on a set of Manitou forks with a 203mm rotor though, so imagine it will fit your setup...
