shoes yes, boots no, although doing boots straight across is a bit weird

Neither of those above but a combination of the two. Laces should be crossed but you should never mix 'ins' and 'outs' when lacing.

If the lace leaves a hole in the outward direction it should re-enter in the inward direction.

Much neater.

The one on the left of the picture – right idea but you've got it all wrong. Have both the bottom laces coming out, then carry on to the top.

It entirely depends on which way you hang your toilet roll.
If it hangs away from the wall then you have your laces straight across.
If it hangs against the wall then criss cross.

Brogues should be straight laced.

Probably wouldn't wear them myself, as they don't fit my style, but I think they're quite cool.
