  • What have I done to my knee?

First decent length road ride for a few weeks on Sunday (not been sat on my arse, just not had time/weather to go out for more than a couple of hours). Near the top of the last climb my right knee suddenly started hurting. Dead centre under the kneecap, just as I drive down with that foot.

Bar a 10 minute pootle to the shop yesterday (intstantly not right), I rested it yesterday. Today I can feel it walking up stairs.

It’s not agony, but the harder I pedal, the more it hurts.

Any guesses what I’ve done? Nothing has changed in my bike setup (got new shoes 2 or 3 months back, but they’ve not given me any niggles). Got a gym session tonight and about 30 miles of commuting between Weds & Thurs to look forward to, and I don’t want to bollox it up any more.

    Thanks!

Your knees keep the bottom half of your legs on, therefore, if in any doubt get yourself checked out. IMO of course.

Rest your knee for a good few days – if you don’t you could potentially mess it up for longer or permanently. At the gym just do upper body stuff, if it doesn’t improve go to the doctors its free you know.

its most likely overuse, nothing to worry about but your GP is the man to ask but odds on he will just recommend rest and ibuprofen.

I’m into my third month of knee pain after twisting it badly climbing. Initially I had swelling and intense pain down the inside of the knee where it joins my lower leg, medial ligament apparently. When that eased I was left with pain under the knee cap (Anterior knee pain). It locks out when I straighten my leg meaning I cant walk normally and it feels very odd.

Doctor is still saying just rest it and take pain killers 3 months in 🙁

Sounds exactly like the tendonitis I’ve been suffering this year. But could be something else entirely of course.

Mine’s largely cleared up or at least reduced to twinges now – but I haven’t done more than 60 miles in one go since June so perhaps it just hasn’t had the chance to reappear. When it flares up I try to focus on MTB (flat pedals) for a bit to lessen the stress.

I saw a sports injury specialist and now use cleat wedges and orthotic insoles to counteract my wonky legs BTW.

@darren a GP is almost certainly going to know the square root of f-all about sports related knee issues, the only benefit of seeing the GP is to ask for a referral to a specialist.

OP I imagine you don’t have any swelling ? If you do take ibuprofen to reduce that, probably too late now to do any icing. Very hard to diagnose on here, you could be developing some ‘nodules’ (small growths) on the back of your knee cap. Knees are very complex and really you need to see a specialist.

    Nope – no swelling.

It actually came on pretty quick. Stomped up Curbar Gap, which stung a bit after 60+ miles (but then it always does and it was muscles not joints), then was pedalling up the A621 to Owler Bar and suddenly realised my knee was quite painful.

Don’t have a GP. Haven’t been near a doctor in what, easily 10 years?

I’ll take it easy, but it ain’t going to get absolute rest. Life simply doesn’t work that way at the mo.

TBH if you’re into sports and fitness, it’s worth feeling out local sports specialists, physios and the like- I never did this til I had a big injury to fix but now i go and see them for little things and just get more qualified advice, wish I’d had this years ago. Even if just for knowing when to rest it, when to work it, when it’ll make no difference.

How well did you stretch afterwards ? Foam roller your quads and IT bands a lot. stretch your hammies and see how it feels.

I had similar, GP diagnosed Tendonitus & rest. I did that for 2 months and nothing got better. Went to a proper sports therapist who said my hips were bent, he straightened them in two visits and I was right as rain. GPs are generally for coughs and colds, if you want to get back on the bike go somewhere recommended and pay them money.

Mine used to be sports specialist so she does. Obviously got a bit lucky there.

I was merely suggesting (in a round about way) that its best to see a doctor..rather than maybe asking a bunch of strangers on a forum 🙂

sounds like you have irritated the joint between your kneecap and thigh bone (patellofemoral joint) It’s one of the most common causes of knee pain and pushing hard on a climb can be a good way to start it off.

I would rest the knee for a week (maybe longer) to let it settle down and then try to keep stress on it low, ie low gears and spin on the hills.

there can lots of reasons why this can happen, tight quads, ITB, weak and tight gluts etc so find a HPC registered physio and get assessed. http://www.physiofirst.org.uk/ is a good place to start.

Irritated mine earlier this year and I pretty much rested it for 4 weeks and then began biking again. Seems to be holding up so far but will be keeping going with the stretches to try and prevent recurrence.

Had the same earlier this year. On my supposedly good knee. Was diagnosed as patella tendinitis. Rested for a week and took some pain relief and was fine. Then the Achilles had a hissy fit.

I have had similar problems due to the kneecap not tracking properly and rubbing on bone. Some more info here:

I had this come on last winter. It was diagnosed as PFPS (as above) and took 2 months of rest and specific exercises to fix it. Touch wood it has been fine in 11 months since with plenty of riding.

It is supposed to be caused by overuse, at the time I put it down to either taking up singlespeeding or doing a few consecutive road rides in some wind/waterproof bibs which don’t stretch anywhere nearly as freely around the knee as normal bibs. I have since done plenty of singlespeeding with no problems, but today is the first time I am going to wear the bibs since!
