    In order for my feet /knees to be comfortable I have to have my shoes loose so that the outside of my foot can tilt downwards otherwise my foot starts to hurt. I know that for road shoes you can shim the cleats to compensate for this but I can’t see the same available for MTB cleats. Any ideas on a fix?

    Specialized make wedges that go inside the shoes. Maybe they would help?

    There are bike fitters who specialise in this kind of thing, was well worth the money for me.

    Those wedges look just the ticket. I did contact Sigma about fitting but they only do road apparently.

    Specialized BG insoles

    ^ those insoles (in maximum cack-footedness option) basically cured my knee pain that I’d been suffering for years.

    Obviously your issues may be different but worth a go if you think they will help you.

    I have a similar problem, Spec blue insoles and underfoot wedges sorted me out a treat.

    Edit, make sure you get the right ones, you need them for Varus not Vagus.

    You can certainly give the inshoe wedges a go. However, I thought most (respected) fitters state these are a diagnostic tool only, rather than a permanent fixture? They certainly take up lots of volume in the front of your shoe.

    A better quick fix might just be a 1mm washer under the inner side of the cleat. Whether it’ll be solid enough to not move is a different matter. You can get wedges for 2 bolt cleats though.

    A better option may be a heel wedge or two.
