Petplan. Expensive compared to others, but very good.
However, we have our dog-thing insured with GreenBee. They are part of John Lewis, & it's underwritten by AXA. Thankfully, we haven't had the need to use it yet, but coverage is as good as PetPlan, only cheaper.
Funnily enough our Harvey is also known as Mr Biscuit, as the kids think his paws smell of digestives, and they do!

Look out for cheaper policies that don't offer continuing cover (so if your dog gets arthritis it'll be left uncovered after 1 year) or some that have claim limits on single conditions (which would have the same result)
A larger excess will make it considerably cheaper if you can afford to self insure the lower cost misshaps - basically considering insurance as protection against the very worst case scenario stuff.
We've just insured our Cockerdoor (lab/cocker spaniel cross), on a compare the market type thing with Argos, £9.50/month ish, and far more comprehensive cover than most of the other cheapy cheap ones.
Pricing also varies wildly depending on your location too.
